Applications are open for an exciting role as Band 6 Acute Patient Pathway Practitioner (formerly ‘Discharge Planning Nurse’) at the major Health facility on the beautiful Island of Guernsey, in the Channel Islands.
This is a non-clinical/hands-on role, open to suitably experienced Registered Nurses or Allied Health Professionals.
Hours are 08:30-17:00 with one half day per week. One Sunday in four, no Saturdays or evening/night duty.
You will join a team of four Band 6 APPPs and Band 7 Manager.
Using your expertise you will oversee the whole patient journey, and lead on the assessment process to determine any risks or requirements for service users in order to facilitate a safe, timely discharge.
This will include supporting the preadmission process for service users prior to elective admission, for example pre operatively.
You will rotate between two wards each three months which comprise;
– A General Surgical ward
– An Orthopaedic Surgical ward
– A mixed Private Medical / Surgical ward
– Two Acute Medicine wards
– A Medical Rehabilitation ward
The Hospital has four operating Theatres, a Day Procedure Unit, a seven-bedded level 3 Critical Care Unit, Maternity, Paediatric and Neonatal Units and Imaging services.
Additional essential relationships will be with the Community and Social Care Teams and Allied Health Professionals all serving the island of Guernsey’s 65,000 residents and tourist population.
Please note; Guernsey has adopted the AfC banding system, but have applied their own, higher salary scales.
The current Band 6 salary is up to £49,449 plus an annual bonus of £1,350 and higher unsocial hours enhancements; 36% evening/Saturday and 72% Sunday/BH.
Annual leave starts at 36 days up to 42 days with full credit for NHS service for annual leave and sick leave.

Registered Nurse or AHP with full UK professional registration.
Appropriate experience to include discharge planning or post-discharge community setting experience.
Excellent engagement and interpersonal skills.
Able to manage own workload and at times a fast and changeable paced environment.
Ability to think ahead and forward plan.
