We are seeking an appropriately-skilled and qualified Advanced Nurse Practitioner to join the Adult Community Rapid Response team on the beautiful Island of Guernsey, in the Channel Islands.
This multi-disciplinary service is a crisis intervention service which aims to prevent avoidable emergency admissions to hospital or care homes.
The service operates 7 days a week.
You will;
– safely maintain the service user in their own home during a period of crisis which can be resolved with additional short-term support.
– bring advanced knowledge and skills appropriate to community care and be recognised as a clinically autonomous practitioner.
– practice alongside the above service to provide patient centred clinical care. This will encompass the skills of assessment, examination, diagnosis and treatment within your scope of practice
– work in partnership with GPs, St John Ambulance service and ED staff to assist in the safe referral and discharge of patients with un-differentiated and undiagnosed presentations within the community.
Please note; Guernsey has adopted the AfC banding system, but have applied their own salary scale.
The Guernsey Band 8A salary range £67,355 to £80,716 plus an annual bonus of £1,605 and higher unsocial hours enhancements; 36% evening/Saturday and 72% Sunday/BH.
Annual leave starts at 36 days up to 42 days with full credit for NHS service for annual leave and sick leave.

Person requirements:
NMC-Registered Nurse
Current or recent Band 7 or higher experience in Primary Care
Masters degree in Advanced Clinical Practice/ANP qualification
Non-medical Prescribing qualification
Evidence of excellent communication skills with particular reference to Multidisciplinary team working, service users, carers and managers liaising with external agencies
